Exposed food preparation observed under dirty ventilation duct in bar area, potential for contamination. Cheese in bain marie in food preparation area was kept at 55 F when it should be 41 F or less. Discard all food in broken bain marie in food preparation area. Temperature measuring device not available or easily accessible in refrigeration equipment to verify proper temperature of equipment. Observed interior and exterior of bread oven in food preparation area, caked with grease and soil. Observed stove in food preparation area, had accumulation of dust, dirt, food residue and debris on non-food contact surfaces. Clean beverage station. Clean vent in food preparation area next to office. Non-food contact surfaces throughout facility, bar and food preparation areas not cleaned frequently enough to prevent accumulation of dirt and soil. Remove or repair all broken or unused equipment. An inspection of a food facility found evidence of insect activity in food preparation areas; however, the facility has a pest control program. Cleaning supplies and personal items stored in hoods over utensils in food preparation areas are possible sources of contamination.

Stains were observed on the hand washing sink in the restroom. The kitchen hand sink was blocked by a large trash can and was not readily available to staff, the bathroom hand sink had no soap, there were two large holes in the ceiling over the back storage area with a large crack running across the ceiling between them.
